Digital Technology, Value-Based Healthcare and Healthy Aging

摘要: 数字技术是推动价值医疗落地、实现包容性健康养老的关键力量。本文以智慧健康养老示范基地建设为准实验,利用2014—2023年中国老年社会追踪调查五期数据评估这一特定数字技术应用模式的价值医疗效应。研究发现,智慧健康养老作为数字技术与养老服务融合的典型实践,显著提升了老年群体的健康水平并有效遏制了医疗费用增长。基于事件研究法的动态效应分析显示,政策效应呈现持续累积特征。机制分析表明,智慧健康养老主要通过四个维度传导其效应:技术渗透,即数字赋能重构健康决策模式;服务整合,即医养协同优化资源配置效率;环境适配,即适老化改造有效降低外部风险;文化驱动,即社会互动激发个体内生健康动力。异质性检验显示,试点政策效应在农村群体、受教育水平更低以及养老保障不足的老年人群中更为显著,彰显了智慧健康养老的普惠性特征。本文为构建数字经济时代下多元智慧健康养老体系提供了理论依据与经验证据。

Abstract: Digital technology is pivotal for implementing value-based healthcare (VBHC) and achieving inclusive healthy aging. This paper evaluates the VBHC effects of China's “Smart Healthy Elderly Care” pilot program, a specific digital application, by using the smart healthy aging pilot base as a quasi-experiment and drawing on five waves of data from the China Longitudinal Aging Social Survey (CLASS). The findings indicate that Smart Healthy Aging Care, a paradigmatic practice integrating digital technologies with elderly care services significantly improves elderly health outcomes while containing medical expenditure growth. On average, the pilot program increases self-rated health of the elderly and reduces medical expenditures. The event study analysis reveals that these effects are persistent and cumulative. The VBHC effects of Smart Healthy Aging are exerted through four mechanisms: technology penetration (reconfiguring health decisions), service integration (optimizing resource allocation), environmental adaptation (reducing external risks), and cultural driver (stimulating health motivation). The heterogeneity test shows that the policy's effects are more pronounced for rural, less-educated, and pension-uninsured elderly people, highlighting its inclusive nature. Our findings provide empirical evidence for designing diversified smart healthy aging systems in the digital economy.
